The 504 Case Manager supports students in meeting their goals, ensures provision of appropriate accommodations and modifications according to the Individual 504 Plan for the student, and manages the overall learning process for students with 504 eligibility.     ESSENTIAL FUNCTIONS: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential duties.
  • Collaborates with general education teachers to ensure they are implementing the 504 Plan recommendation for success of students;
  • Oversees development and implementation of 504 Plans on attainment of academic standards defined by the State ;
  • Collaborates with students/learning coaches on implementation of 504 plan;
  • Evaluates and reviews student progress on a regular basis and annually;
  • Facilitates 504 meetings timely and according to applicable protocols;
  • Facilitates the revision of the 504 Plan goals as necessary and maintains 504 files, ensuring ensures they are compliant with local and State regulations;
  • Inputs student data in appropriate systems and maintains other student records as needed in a compliant manner;
  • Participates in regularly scheduled staff meetings, professional development (virtual and face-to-face) and school committees;

Requirements
Minimum Qualifications
  • Bachelor's degree in Education or similar area of study AND
  • Four (4) years of related work experience OR
  • Equivalent combination of education and experience

Certificates and Licenses:
  • Teacher Certification-Specialization in 504/Dyslexia
OTHER REQUIRED QUALIFICATIONS:
  • Understanding of applicable sections of the State education laws and other pertinent regulations;
  • Communicates effectively both orally and in writing;
  • Demonstrates the use of good judgment in decision-making;
  • Great organizational and time management skills;
  • Proficiency in Microsoft Excel, Word and Outlook as well as Internet research methods and report writing techniques; utilizes computer software associated with curriculum;
  • Ability to travel 20% time for work including meetings, testing and professional development;
  • Ability to pass required background check including obtain fingerprint clearance where applicable
